Community Care Coordination

 

Care Managers work with clients to coordinate their care using a person-centered approach, connecting clients to resources and services, coordinating plans of care, and providing advocacy and support.

Care Managers consider the needs of their clients so that they may receive assistance, support and referral relative to physical and behavioral health needs, social or community involvement, legal and housing needs.
