Permanent Supportive Housing Program
The Permanent Supportive Housing (PSH) Program is a supportive living program for individuals who have both a substance use disorder and a history of housing instability.
This program is funded through the New York State Office of Addiction Services & Support (OASAS).
Participation
Participants work closely with a Housing Case Manager who assists them in developing long-term, stable housing and employment. Participants must meet admission requirements including diagnosis, risk of homelessness, and income requirements.
Length of Stay
Length of stay is determined on an individual basis. Although this varies, a typical stay is approximately 12 months.
Program Expectations
Residents agree to engage and participate in service planning and treatment, maintain sobriety, and become involved in either employment or a regularly-scheduled volunteer activity.
Eligibility
This program serves adults who have been diagnosed with a chemical dependency or addiction disorder and have a history of housing crises.

Homeless or at risk of being homeless

Admission requirements for PSH-MRT:
* An Axis I Substance Use Disorder
*Residing alone
*Medicaid
*Homeless or at-risk for homelessness
*2 inpatient hospitalizations in a year or 5 Emergency Room visits.
